The entire process of the embryo implantation remains poorly understood, but two factors are certainly required: endometrial receptivity and synchronization of embryo and endometrial development.
Within the natural cycle, those factors are caused through the synchronized development of the follicle and the hormonal events that lead to ovulation. Within the donor egg IVF, those events are obviously separated and have to be controlled and synchronized with a sequence of ovarian down-regulation and endometrial preparation. Due to those factors, controlled timing of follicle growth, egg maturation and ovulation in egg donor, as well as sufficient stimulation of the endometrium with an estrogen-progesterone sequence in the recipient have to be performed.
Generally, recipients are monitored a few times with an ultrasound to determine the endometrial thickness. This enables fertility specialist to regulate the medications when the lining is not sufficient.
